Name Changes in Passports

This page contains special requirements for changing your name in your passport.  These documents are required in addition to other requirements for your specific passport service.


In order to change your name in your passport, you must apply for a new passport.  The U.S. Embassy does not amend passports.


Applications for a change of name must be accompanied by the following documents:


One of the following:

  • Original, Official Marriage Certificate
  • Original, Official Divorce Certificate
  • Original, Official Name change document issued by the U.S. or foreign government

Or

Three of the following that have been valid for at least 5 years:

  • Driver's License
  • Israeli ID card
  • Foreign Passport
  • Other official picture ID’s
